Submitting a DMCA Notice
To file a DMCA notice with us, please send your complaint via email to dmca@livingwilltemplate.com. Your email should include the following: a detailed description of the copyrighted work that you believe has been infringed upon, a link or description of where the alleged infringing material is located on our site, your contact information (such as address, telephone number, and, if available, an email address), a statement by you, under penalty of perjury, that the information in the notification is accurate, and that you are the copyright owner or authorized to act on behalf of the owner.
Action Upon Receipt of a DMCA Notice
Upon receiving a DMCA notice, Living Will Template will take whatever action it deems appropriate, which may include the removal of the challenged content from the website. We will then promptly notify the individual responsible for posting the content, allowing them the opportunity to respond with a counter-notification, if they believe the removal was a result of a mistake or misidentification.
Counter-Notification
Should you deem the removal or disablement of the material to be the result of a mistake or misidentification, you may submit a counter-notification. This counter-notification must contain your signature (either physical or electronic), identification of the material that has been removed or to which access has been disabled, a statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of a mistake or misidentification, and your full name, address, telephone number, and, if available, an email address, along with a statement consenting to the jurisdiction of the Federal District Court for the judicial district in which your address is located, or if outside the United States, any judicial district in which the service provider may be found.
